Support Coordination

Support Coordination is an NDIS funded support that is designed to support you to make the most of your NDIS funds.

The delivery of Support Coordination is to assist in strengthening a person’s ability to design and build their supports with an emphasis on linking the broader systems of support across a complex service delivery environment.

Support Coordinators focus on supporting people to direct their lives, not just their services. This involves working together to understand the funding, identify what people expect from services, and how individuals want this designed. Support Coordination also includes coaching and working with the person to develop capacity and resilience in their networks.

The intended outcomes of providing Support Coordination is that a person’s capacity and ability to design, develop and implement their supports is sustained and includes, but is not limited to:

Connection – assisting the person to develop knowledge, experience and connections with the community and broader systems of support.

Support Design – working together with the person to understand plan funding and its purpose. The Support Coordinator will strive to understand the person’s confidence and skills, and assist the person to identify what they want from services.

Establish Supports – assisting the person to identify and consider support options and link them to the broader systems of supports. Where practical, to create a support and action plan to facilitate the person to implement their plan.

Crisis – assistance to resolve points of crisis and develop capacity and resilience in the person’s network.

Coach, Refine, Reflect – coach the person through challenges that come up and assist the person to prepare for review and report on outcomes achieved.
